Participation Waiver & Release

1. Who this covers

This Participation Waiver & Release ("Waiver") is an agreement between the parent or legal guardian who signs it ("I" or "me"), on my own behalf and on behalf of my child named in the registration ("my child"), and Beyond the Spectrum Sports, a Nevada nonprofit corporation, along with its officers, directors, coaches, staff, volunteers, sponsors, and the owners and operators of the fields and facilities we use ("Beyond the Spectrum Sports").

2. Voluntary participation

I am the parent or legal guardian of my child. I am choosing to let my child take part in Beyond the Spectrum Sports soccer practices, games and related activities ("activities"). Participation is voluntary.

3. Understanding the risks

I understand that soccer is a physical sport and that taking part in the activities involves risks, including but not limited to falls, collisions with other players, contact with balls, goals or other equipment, uneven playing surfaces, heat, sun and weather, and injuries such as sprains, broken bones, concussions and, in rare cases, more serious injury. I understand that these risks cannot be completely eliminated, even with careful supervision. I knowingly and voluntarily accept these risks on behalf of myself and my child.

4. Release of liability

To the fullest extent allowed by Nevada law, I release, waive and agree not to sue Beyond the Spectrum Sports for any injury, illness, loss or damage to my child or to me that arises from or is connected to my child's participation in the activities, including claims based on the ordinary negligence of Beyond the Spectrum Sports. This release does not apply to gross negligence or intentional misconduct.

5. Health and fitness

I confirm that my child is physically able to take part in the activities. I agree to tell Beyond the Spectrum Sports about any medical condition, allergy or other information that would help my child take part safely. I understand that Beyond the Spectrum Sports is a sports program, not a therapy program, and does not provide ABA therapy, medical care or treatment.

6. Emergency medical care

If my child is injured or becomes ill and I cannot be reached right away, I authorize Beyond the Spectrum Sports to call emergency services and to arrange first aid or emergency medical care that it believes is needed. I understand that I am responsible for the cost of any medical care my child receives.

7. Supervision and rules

I agree that a parent or guardian will stay on site during every practice and game. I agree that my child and I will follow the instructions of coaches and staff and the program's rules. Beyond the Spectrum Sports may ask a player, parent or spectator to step away from an activity or leave the program if their behavior puts anyone's safety at risk.

8. Photos

Beyond the Spectrum Sports will only post photos or videos of my child according to the photo permission I choose when I register.

9. Electronic signature

I agree that checking the waiver box and typing my full legal name during registration is my electronic signature, and that it has the same effect as signing this Waiver by hand.

10. General terms

This Waiver is governed by the laws of the State of Nevada. If any part of it is found to be unenforceable, the rest stays in effect. This Waiver applies to the season I register for and to any later season my child takes part in unless I sign a newer version.

I have read this Waiver, I understand it, and I am signing it voluntarily. I understand that I am giving up certain legal rights by agreeing to it.
